Please answer the following questions:     Are you undertaking a full time undergraduate/postgraduate course of two years or more at a UK college or university?   YES NO   Do you know your term time address?   YES NO Have you at any address suffered any loss, damage or liability during the last three years, whether you were insured or not?   YES NO Have you ever been convicted of, or do you have any prosecutions pending on any offence? (It is not necessary to tell us about driving offences for parking or speeding, or any offences which are spent under the Rehabilitation of Offenders Act 1974).   YES NO Have you received a formal police caution in the last 5 years?
